The assisted living facility last sold for $7 million in 2014, records show.
The building was sold to investment company KNC Capital Partners.
The hospital, owned by Nashville, Tennessee-based HCA Healthcare Inc., bought the property from the Vaughn Family Trust in December.
The new building is expected to be completed in the first quarter of 2025.
One Health Medical Centers has proposed a two-story medical office of 21,670 square feet, including patient exam and treatment rooms, as well as imaging, physical therapy and dentistry.
